How to create the first page? All options grayed out, and "new item" missing.

I could not find any tutorials, but I have tried following the "gettings sarted" documenation section, which has a "create starter project" guide. I cant link to it, as this forum page always crashes when I submit the page with the link.

I created a blank project, and now I need to create a page.  The above article says:

1. In Visual Studio, go to the Models folder, and right-click on Pages.
2. Select Add > New Item, then Episerver and Page Type. 

The problem is under add there is no "New Item". There is only:

•   Controller (Greyed out)
•   Existing Item (greyed out)
•   New scaffolded item (greyed out)
•   New folder (greyed out)
•   Add ASP.net folder
•   REST api client
•   Client side library
•   New Azure wep project
•   Existing project as Azure web job
•   Class (greyed out).

Any idea how I create a new page?

NOTE: I cant access the /episerver CMS ui, as I dont know the login.
